Applications for Passports may be obtained in person at the St. Martin Parish Clerk of Court's Office, or you may download them below.

Passport applications are accepted Monday through Friday from 8:30 a.m until 3:30 p.m.​
​
Due to the high volume of applications currently processed by our office, we ask that you call 337-394-2210 prior to arrival to check availability so we may serve you efficiently.
U.S. Passport Book
Valid for travel by air, sea or land.
​​​Routine processing time is 6-8 weeks.
You will need the following to obtain a passport:

A certified copy of your birth certificate or proof of U. S. Citizenship

Proof of identity; a valid driver's license, military ID card, government ID card

Two (2) color passport photographs (2" X 2")

Check or money order to: "U.S. Department of State" in the following amounts

$130.00 for adults, age 16 and over (valid for 10 years from date of issue)

$100.00 for children, age 15 and under (valid for 5 years from date of issue)

Separate payment of $35.00 for application fee, payable by separate check, money order, or cash is due to the Clerk of Court
U.S. Passport Card
Customers now have the option to apply for a Passport Book, a Passport Card, or Both. The Passport Card may only be used for land and sea travel between the United States and Canada, Mexico, Bermuda, and the Caribbean. The Passport Card is not for travel by air. The Passport Card will provide a less expensive, smaller, and more convenient alternative for those who travel to these destinations. To apply for a Passport Card, use the same application forms used for a Passport Book. Routine processing time is currently 6-8 weeks.

You will need the following to obtain a passport:

$30.00 for adults, age 16 and over (valid for 10 years from date of issue)

$15.00 for children, age 15 and under (valid for 5 years from date of issue)

Separate payment of $35.00 for application fee, payable by separate check, money order, or cash is due to the Clerk of Court if applying for Passport Card only. If applying for Passport Card at the same time as Passport Booklet, the $35.00 application fee is waived for the Passport Card. (Total fee for both would be $160.00 to Passport Services plus $35.00 to the Clerk of Court.)

Passport Renewal (by mail)
For details on Passport Renewals, please click on the link below. Passport Renewals are to be mailed directly by the applicant to the National Passport Processing Center. You must use the United States Postal Service (USPS) to mail your renewal application and supporting documents.​​​​​
​​​​​Routine processing time is 6-8 weeks.

Fee is $130.00 payable by check or money order to "U.S. Department of State"
You may qualify to use this form only if:

The issue date of your last passport was less than 15 years ago;
-
You were 16 or older when it was issued;
-
Your name is the same or you can submit name-change documentation (marriage certificate or court order);

You can submit your current, undamaged passport

You will need to submit the following with your completed, signed renewal form to renew your passport:
- Current undamaged passport (issued less than 15 years ago)
- One color passport photograph (2" X 2")
- Check or money order to: "U.S. Department of State" in the following amounts:
- $130.00 for adults, age 16 and over (valid or 10 years from date of issue)​

Customer must arrange for 1-2 day delivery to ensure receipt within 9 weeks.

If international departure in less than 2 weeks, or passport is needed for foreign visas, application at a Regional Passport Agency may be advised. An appointment is required.
